Service Coordination (SC)
We help you access the services and supports you need, while guiding you through your goals and choices.
Individualized Skill Development (ILST)
We provide hands-on support to help you build daily living skills, increase independence and reach your goals.
Community Transition Services (CTS)
We support you as you transition from an institutional setting to your home and community.

Who We Serve
Individuals & Families
We work with individuals who are eligible for the NYS TBI and NHTD Waiver programs, as well as their families and caregivers, to provide the support and guidance needed to live as independently as possible.
